The ‘Transition Roadshow’, is a partnership event hosted by Swansea Carers Centre and Careers Wales. The Transition Roadshow creates a marketplace of all post 16 options for young people with a range of additional needs and is the chance for parent, carers, professionals and students and is the opportunity to come and find out the range of opportunities and choices that they have when leaving school.
