All pupils participating in extracurricular PE activities should have received a consent form from the teacher leading the activity. If pupils have not yet collected a consent form, please could you ask them to come to the PE department office to collect a consent form as soon as possible.
They will need a consent form for each sport, so that we can ensure that staff have the correct information when taking pupils out of school.
