The Physical Education department would appreciate your help in ensuring that we have the details of your son/daughter in order that we are able to contact you quickly in the event of an accident/ incident during any extracurricular match or training session.

This consent form will cover all matches fixtures and training sessions throughout the current academic year
The department has also started a twitter account @Gowertonpe this will enable you to keep up to date with fixtures, cancellation/ postponements of matches , return times from fixtures plus match results.
